  • Please ensure you are prepared for service with a completely clean face and prepped with your personal skincare products (if not available we can prep your skin before makeup application).

  • Removal of previous makeup will result in less time for application and may result in additional fees and overage in time at the client’s expense.

  • Reminder to wear a button down top or loose fitted shirt as to not disturb application once it is applied.

  • Brow shaping and grooming is recommended to be done at least 1 week prior to your event. Service provider is not responsible for brow shaping on day of event.


The client does hereby consent and agrees that Tabitha Price Makeup (Tabitha Price) has the right to take photographs or digital recordings of me, the client, during my Makeup Application and or during my photo shoot. These photos/videos may be used for promotion purpose across all social media platforms, including but not limited to Instagram, Facebook and www.tabithapricemakeup.co.uk The client further consents that their name and identity may be included therein or by descriptive text or commentary.


  • The client acknowledges the contagious nature of the Coronavirus/COVID-19 and that the GOV.UK website and many other public health authorities still recommend practicing social distancing where possible.

  • The client further acknowledges that Tabitha Price Makeup (Tabitha Price) has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I, the client, acknowledge that Tabitha Price Makeup (Tabitha Price) CAN NOT guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to personal service providers, and receiving personal services such as I am entering in with Tabitha Price Makeup (Tabitha Price).

  • I, the client, voluntarily seek services provided by Tabitha Price Makeup (Tabitha Price) and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.

  • I, the client, attest that:

* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not travelled internationally within the last 14 days.
* I have not travelled to a highly impacted area within Canada in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I am following all Governmental Health recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I, the client, hereby release and agree to hold Tabitha Price Makeup (Tabitha Price) harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Service Provider, or that may otherwise arise in any way in connection with any services received from Tabitha Price Makeup(Tabitha Price) I understand that this release discharges Tabitha Price Makeup from any liability or claim that I, my heirs, or any personal representatives may have against the Service Provider with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from the Service Provider. This consent and liability release extends to Tabitha Price Makeup (Tabitha Price) and any associates of said Service Provider.


All brushes, tools, and makeup products are sanitized between every makeup application. Makeup products used are hypoallergenic. Any allergies and/or skin conditions or illness would, could impede makeup application or risk the health of the Service provider or any associates of the Service provider must be reported by the client to the Service Provider prior to application and, if need be, a sample test of makeup may be performed on the skin to test reaction.

  • Client(s) agrees to release Tabitha Price Makeup (Tabitha Price) and any associates from liability for any skin complications, allergic reactions or illness caused from or due to makeup application or services provided.

  • The client agrees to release Tabitha Price (Tabitha Price) and any associates from liability for any damages accidentally caused during the service to personal property or belongings of the client.

I, the client, have read and fully understand this agreement and all information detailed above. I understand the service and accept the risks. All of my questions have been answered to my satisfaction and I consent to the terms of this agreement. I / we have read this letter describing Fresh Face Artistry services and agree that it reflects the discussion I had with her and my intention to participate full in this arrangement. I / we understand that life & business are unpredictable and will not hold Rachel Sylvester, Fresh Face Artistry liable regardless of the outcome. By agreeing here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.





Cancellation Fees

More than 26 weeks before the Event Date – 10% of the total cost

From 181 to 84 days before the Event Date – 25% of the total cost

From 83 to 28 days before the Event Date – 50% of the total cost

From 27 to 10 days before the Event Date – 75% of the total cost

From 9 to 3 days before the Event Date – 90% of the total cost

Less than 3 days before the Event Date – 100% of the total cost


To secure your event date a 50% deposit will be invoiced to you prior to your event date.
Please note the deposit is non-refundable.
